Wellspan EAP is not accepting new affiliate provider applications at this time.
IMPORTANT NOTICE
After a comprehensive market assessment, WellSpan has decided that it will no longer directly provide EAP services. WellSpan will, however, continue to provide EAP services to employers through a subcontract with Quest Behavioral Health, Inc. (“Quest”), effective September 1, 2024. You will be receiving a notification letter shortly if you have not already received one.
WellSpan EAP will continue to process claims for the services you provide before September 1, 2024. You must submit to WellSpan all claims for services provided no later than sixty (60) days after you complete service. Claims that are not timely submitted will be denied.
You must retain the records for individuals to whom you have provided services for the period required by applicable federal and state laws; however, in no event shall the records be retained for less than seven (7) years. Your obligation to preserve these records does not terminate with the termination of the Agreement.
If you are not currently credentialed with Quest EAP/Quest, and to ensure continuity of care for clients, we encourage you to please reach out to Quest at provider@questbh.com. Please provide your name, licensure, date of birth and email contact and they will send an online application.
